Job Overview

The Clinical Specialty Coordinator is a Registered Nurse (RN) who coordinates and oversees a program specific in a defined specialty population. The Clinical Specialty Coordinator works across the continuum to develop, implement, and support quality improvement efforts that improve outcomes for the specialty practice. Areas of focus include program development, standardization of practice and documentation, communication, sharing of information, hand-offs, transitions, advocacy, patient and family guidance, education, resources, and specialty specific expertise. The Clinical Specialty Coordinator provides guidance and support to staff in the management, provision and coordination of care in their area of expertise.

Qualifications

Education: Bachelor of Science in Nursing (BSN)

Experience: Four years of nursing

Certification(s):

  • BLS/CPR from the American Heart Association with at least 6 months left before expiration is required upon hire.
  • Achievement and maintenance of certification in one of the two categories:
    • Certified Case Manager (CCM) through the Commission for Case Manager Certification (CCMC), Accredited Case Manager (ACM) through the American Case Management Association (ACMA) or certification in Care Coordination and Transition Management (CCCTM) through the American Nurses Credentialing Center (ANCC).
      • A plan to obtain certification will need to be in place with the team member's direct leader within 6 months of hire with the expectation that the certification exam will be taken once all accrediting body eligibility requirements are met.
    • ANCC or Magnet approved clinical specialty certification inclusive of care coordination activities as approved by the Director of Patient Care Services.
      • A plan to obtain certification will need to be in place with the team member's direct leader within 6 months of hire with the expectation that the certification exam will be taken once all accrediting body eligibility requirements are met
  • Successful completion of required internal training within six (6) months of hire or transition into role

Licensure(s): Registered Nurse (RN) OR Registered Respiratory Therapist (RRT) licensure in the state of Colorado is required.

Professional Membership(s): Active membership in a professional nursing or healthcare organization is required.

POPULATION SPECIFIC CARE

  • Neonate - <30 days

  • School age - >5yrs to 13yrs
  • Adolescent - >13yrs to 18yrs

ESSENTIAL FUNCTIONS

An employee in this position may be called upon to do any or all of the following essential functions. These examples do not include all of the functions which the employee may be expected to perform.

  • Coordinates program specific care by:
  • assessing patient/family needs while obtaining treatment in specific program
  • establishing and implementing program specific Care Plan in partnership with the patient/family and multi-disciplinary team to proactively support the patient/family
  • connecting families back to primary care provider and Clinical Care Coordinator, if applicable
  • documenting within the standardized tools and maintaining the Care Plan
  • evaluating patient/family needs and outcomes to continuously update the Care Plan
  • tracking care coordination interventions and outcomes.
  • Develops and maintains data system to track patients, interventions, and outcomes within the specialty.
  • Supports staff in standardized practice and documentation, and stream-lining data tracking and reports.
  • Participates in the development and revision of policies and procedures and quality improvement and research projects impacting the care for the specialty group.
  • Builds relationships with other like pediatric programs to collaborate and share data that has benefits to the larger pediatric specialties across the region and nationally.
  • Maintains all program specific national standards and requirements of accrediting organizations including collection and reporting of data to appropriate national registries and databases.
  • Develops and delivers presentations and other materials for use during program-related events and meetings.
  • Provides education to the healthcare team on the management of specific patient needs.
  • Utilizes multiple forms of communication to collaborate with patients, families, and other care team members across the continuum of care as well as with quality improvement and research activities, impacting care in designated program.
  • Participates in an internal learning community and professional governance committees focused on care coordination or population health.

  • Supervision Received: Performs work with minimal supervisory oversight. Work assignment is generally unstructured and employee is responsible for organizing complex, varied, and simultaneous coordination of several functions, programs, or projects in various stages of completion.

Salary Information

Annual Salary Range (Based on 40 hours worked per week): $78,659.24 to $117,988.86
Hourly Salary Range: $37.82 to $56.73
