Position classification

Hybrid -- Job duties and expectations allow for onsite and remote work scheduled every week. Employees in this classification are regularly scheduled onsite one (1) to four (4) days per week based on the County needs and as determined by Department Director. Hybrid classified roles can be onsite more than the set minimum based on employee preference.

What Success Looks Like In This Job

This position is in the Adams County Health Department (ACHD) Division of Health Equity and Strategic Initiatives (HESI).  The Housing Stability Coordinator is a pivotal new role within Adams County Health Department focused on housing stability and health.  This position integrates the importance of residents having access to stable, affordable housing as a social determinant of health.  Through connections with navigators working with people in communities to assure connections to resources, this position boosts the health and economic stability of Adams County residents by creating greater systems of care and resources for them.  
 
The Housing Stability Coordinator is funded through a Colorado Department of Public Health and Environment COVID 19 Health Equity Grant through May 30, 2024.  This position will work with Adams County Health Department (ACHD) COVID19 Equity staff, Adams County Department of Community Safety and Wellbeing Homeless Navigators and others serving people experiencing housing instability to understand the experiences of people at risk of housing instability/experiencing homelessness during COVID19 pandemic to identify the gaps in systems of care and work with community partners to address them.  Partnerships will include local government, community based organizations, and housing providers to identify, analyze, develop, and/or implement policy, systems, and environmental changes that promote housing stability and access to community resources.  The Housing Stability Coordinator will facilitate work related to establishing community partnerships with diverse stakeholders, integrating housing stability and access to resources into existing systems and businesses, integrating housing stability and access to resources into local land use planning and policy, and informing local, state and federal policy related to housing stability and access to community resources with a focus on equity and justice.  

Examples of Duties for Success

Leadership:  Conducts the following to increase access to health through housing stability and community resources:  

  • Serves as central point of contact and resource with deep understanding of housing stability and access to community resources in relation to health and provides technical assistance, as appropriate, for health department staff, local government staff, elected officials, community organizations and the public.
  • Facilitates program efforts to assess local housing and care navigation systems, which includes leading teams to collect, document, and analyze information about the existing conditions of an area.
  • Communicates effectively, both verbally and in writing, to inform, educate and advise the community, elected officials and government officials 
  • Contributes to the development of educational models and materials to promote policy, system and environmental changes 
  • Communicates effectively, both verbally and in writing, to make informational, educational, and persuasive presentations internally and externally.
  • Gather, use and consider the application of data to understand and solve problems.
 
Collaboration and Community Engagement:  Conducts the following to access to health through housing stability and community resources:  
  • Convenes and facilitates Adams County partners, including community members, to identify and facilitate actionable work toward a shared community goal.  Plans, prepares and facilitates meetings, with interactive presentations, agendas, speakers, and community leaders.  
  • Based upon identified community needs, assists with development and evaluation of community based initiatives and forges partnerships with key partners to advance the initiatives within the community.
  • Develops and maintains positive, trusted working relationships with community members, community partners, ACHD staff, and  Adams County staff in order to advance work in a mutually beneficial manner.  
  • Participates in regional and state-level workgroups to influence regional and state initiatives to meet Adams County residents’ needs.  
  • Establishes and maintains positive working relationships with Adams County Health Department staff, outside agency staff, policy makers, community partners and the public.
 
Policy and Systems Changes:   Conducts the following to access to health through housing stability and community resources:  
  • Gathers and analyzes information from community members and organizations that can inform options for policies, programs, and services and the related housing stability and access to resources.   Advises the development of policies, programs, and services at various levels of government as possible.  
  • Utilize systems-oriented approach to understand the multiple origins of a complex challenge and how the parts of a system can work together to improve community outcomes.
  • Participates in regional and state-level workgroups to influence policy in order to meet Adams County residents’ needs by integrating public health considerations into decision-making.      
 
General/Administrative:  Conducts the following to access to health through housing stability and community resources:  
  • Identifies and analyzes quantitative and qualitative data and information that can be used to understand health and housing access 
  • Conducts research of evidence-based findings that can impact policy and program decision-making.
  • Uses information technology in accessing, collecting, analyzing, using, maintaining, and disseminating data and information.
  • Assists and leads in the preparation of reports and program evaluation by gathering information, describing relevant evaluation data, and preparing documents
  • Participates in program development, grant writing, and grant implementation, as appropriate, to advance the work in the community 
  • Prepares written materials and presents program updates in for the public, other government agencies and elected officials 
  • Responsible for monitoring compliance with applicable program contracts, monitors budgets, expenditures and billing processes of assigned program.
  • Coordinates activities and engagement with partners in alignment with other ACHD and Adams County assessment and planning efforts.   Identifies opportunities to leverage existing efforts to best serve community.

  • Participate in all exercises and drills on emergency preparedness, as required.
  • Complete emergency preparedness and response trainings as appropriate for this level of employee within the health department.
  • Maintain sensitive & confidential client information according to the HIPAA policy and Title X confidentiality requirements.

Qualifications for Success

  • The socioecological model of behavior change and how environmental, social and policy influences on health align to create change 
  • Social determinants of health, their impact upon health, and strategies to address them within community settings 
  • Techniques and best practices related to community engagement, coalition building and developing community partnerships.
  • The integration between housing stability and community settings 
  • Community needs assessment, program development, and evaluation techniques.
  • Community planning elements that protect public health and facilitate healthy behaviors and access to basic services
  • Microsoft operating system computer, Internet and spreadsheet functions and resources.

More Qualifications for Success

Any combination of experience and training that would likely provide the required knowledge and abilities is qualifying. Years of experience may substitute for a degree, on a year to year basis. A typical way to obtain the knowledge and abilities would be as follows:

  • Requires a Bachelor’s Degree from an accredited college or university in public or environmental health, public policy, sociology, urban or transportation planning, or a related field. 
  • Minimum five years prior experience in the following:
    1. Working with coalitions, local government, policy makers and/or government entities to measurably improve health and social outcomes 
    2. Leading program planning to achieve successful, sustainable, policy and systems-level changes  
  • A Bachelor's Degree will substitute for 4 years of experience
  • Experience in housing stability and health and housing preferred.  Lived experience is recognized as experience.    
  • Master’s degree preferred.
  • Bi-lingual preferred.  
 
Special Qualifications:
Must be in possession of a valid driver’s license and access to dependable transportation for business purposes as needed. Must be proficient in use of online meeting platforms and Microsoft Office tools including Word, Excel, PowerPoint, and Outlook.

Working Conditions and Physical Requirements
  • Work is primarily conducted indoors with occasional time spent in an outside environment. Incumbent will experience routine exposure to hazards of a typical office environment. Requires travel between Adams County offices and to locations in the surrounding community for investigations and meetings. May be required to work outside regular hours to attend meetings or events.
  • Occasional moderate lifting and carrying objects up to 15 pounds.
  • Occasional use of automobile, usually no more than one hour at a time.
  • Requires sufficient hearing and speech ability to communicate verbally in response to inquiries, to conduct needs assessments and evaluations, and to speak to groups of people in presentations.
  • Requires sufficient writing ability to fluidly and accurately communicate needs in grants and program development.
  • Requires manual dexterity to operate computer equipment
  • Requires ability to occasionally drive an automobile in all types of weather conditions.
